What is Abilify (Aripiprazole)?

Abilify is an atypical antipsychotic used to treat schizophrenia, bipolar disorder, major depressive disorder (as an add-on treatment), irritability associated with autism spectrum disorder, and Tourette's disorder. Unlike older antipsychotics that broadly block dopamine receptors, Aripiprazole acts as a partial agonist—meaning it both activates and moderates dopamine activity depending on whether levels are too high or too low. This nuanced mechanism is why it's often better tolerated than other antipsychotics, with less risk of weight gain, sedation, and metabolic effects.

Partial dopamine agonism

Acts as a partial agonist at D2 receptors—stabilizing dopamine activity by activating receptors when dopamine is too low and competing with excess dopamine when it's too high, producing more balanced neurotransmitter regulation.

Serotonin modulation

Also acts as a partial agonist at 5-HT1A receptors and an antagonist at 5-HT2A receptors, contributing to its effectiveness for depression and anxiety symptoms alongside its antipsychotic action.

Favorable tolerability profile

Less likely than many antipsychotics to cause significant weight gain, sedation, or metabolic changes—making it a preferred option for long-term maintenance treatment in conditions like bipolar disorder and schizophrenia.

Frequently Asked Questions

How long before Abilify starts working?

For acute mania or psychosis, some symptom improvement may appear within the first week. For depression as an add-on treatment or for maintenance in bipolar disorder, full benefit may take 4–6 weeks.

Is Abilify weight-neutral compared to other antipsychotics?

Aripiprazole has a more favorable metabolic profile than many other atypical antipsychotics — it's less likely to cause significant weight gain, elevated blood sugar, or lipid abnormalities that are common concerns with long-term use.

Can I stop Abilify on my own?

No. Stopping antipsychotic medication abruptly can trigger withdrawal symptoms and increase the risk of symptom relapse — especially for schizophrenia and bipolar disorder. Always taper under your clinician's supervision.

What side effects might I experience?

Restlessness (akathisia), nausea, insomnia, headache, and dizziness are most common. Aripiprazole is less likely to cause drowsiness than many other antipsychotics, which some people find an advantage.

Is Abilify safe for use in adolescents?

Yes. Aripiprazole is approved for schizophrenia and bipolar disorder in adolescents aged 13–17, and for irritability associated with autism spectrum disorder in children aged 6 and older.
